2022 Winner: Cardiff and Vale University Health Board - 3D Optometrist training collaboration
Cardiff and Vale’s five-year plan, in partnership with Cardiff University, provides training and clinical placement opportunities for optometrists which enables them to provide a wider range of services in their practice closer to the patients’ homes. This frees up essential hospital capacity so that patients with critical eye conditions are seen in a timely manner, potentially reducing irreversible damage to their eyesight.

NHS Kernow Clinical Commissioning Group and Cornwall Council announced an agreement in February for a new approach to working together on social care, health commissioning and public health services. The new joint ‘Accountable Officer’ role will incorporate the duties currently undertaken by Cornwall Council’s Strategic Director of Adult Care and Health and those of NHS Kernow’s Chief Officer to provide joined-up leadership to deliver on the new vision for Health and Well-Being for the next decade.

Lancashire Teaching Hospitals NHS Foundation Trust’s Technology Services team have collaborated with the Inspira Group and Lancashire Enterprise Advisor Network to inspire the future NHS workforce. A large group of the Technology Services team, including colleagues from the Digital programme, Technical Infrastructure, Projects, IT Training, Information Governance and Management teams attend high schools in Lancashire and participate in careers workshops with young people. The team have promoted the importance and wide range of careers in Health Technology and so far, have visited over 20 high schools, reaching over 1,000 students by attending various digitally‐inspired activities.

This innovative new facility, the first of its type in the UK, has the potential to transform care for older people in Hull by providing out of hospital care, reducing unnecessary hospital admissions and enabling residents to keep fit and healthy and live independently in their own homes. A new clinically led team will primarily treat 12,000 frail older people in Hull with long term conditions who have been assessed by their GP as being at risk of hospital admission. The longer term aim of the centre, which sits on the site of the former David Lister School in East Hull, is to reduce unplanned admissions by up to 20 per cent and is forecasted to save the NHS up to £5.8 million.

Safe Drive Stay Alive is a partnership between Greater Manchester’s emergency services, Salford Royal, and bereaved family members that aims to reduce the number of young driver related deaths and injuries on our roads. The performances use a mixture of live speakers and film to encourage young people face up to the realities of careless driving and to realise the huge responsibilities that a driving licence brings with it.

Yorkshire Ambulance Service NHS Trust has joined forces with South Yorkshire police and firefighters with the aim of reducing the demand on 999 responders. The Local Intervention and Falls Episodes (LIFE) team will attend lower priority incidents in Sheffield, seeing fire and police staff visit hundreds of homes in the city to reduce fire risk in properties, improve security and help people who have fallen and contribute to reducing the risk of falls. The team operates using two specialist vehicles and four staff - two South Yorkshire Fire & Rescue employees and two South Yorkshire Police community support officers (PCSOs).
